1) public transport;
2) kinds of specialists;
3) number of dentists;
4) family discounts.

You have 20 seconds to ask each question.

Можно, к примеру, задать следующие вопросы:

1) What public transport options are available to get to the clinic?

2) What kinds of specialists work at your clinic?

3) How many dentists do you have on staff?

4) Do you offer any family discounts?

Вопросы по содержанию должны отвечать поставленной задаче и иметь правильную грамматическую форму прямого вопроса.
