1) location;
2) public transport;
3) dentist;
4) family discounts.

You have 20 seconds to ask each question.

Можно, к примеру, задать следующие вопросы:

1) Where is the clinic located?

2) What public transport options are available to get to the clinic?

3) Is there a dentist in your clinic?

4) Do you offer any discounts for families?

Вопросы по содержанию должны отвечать поставленной задаче и иметь правильную грамматическую форму прямого вопроса.
